9:00 a.m.- 4:30 p.m. Mon-Fri.If you are in need of a time sheet, you can print the form located under the "Home & Community-Based Services" tab, or you may call our

office to request a time sheet via fax, email or U.S. Postal mail. You may reach us by calling (313) 963-8383. To submit a timesheet or any other documents,
